To check eligibility, first of all go to the official website of PM Jan Arogya Yojana.

After this, you have to enter your 10 digit mobile number here

When you have filled the mobile number, you will see a captcha code on the screen, fill it here

Then you will see the option of Generate OTP on which you have to click

After this, you will see that an OTP has come on the mobile number you have filled and you have to enter this OTP here

Then you have to choose your province, as well as your district

Now you have to fill your name, father's name and all other required information here

After this you will be able to know whether you are eligible to get Ayushman card or not.
