First, you need to visit the Ayushman Mitra help desk at the hospital. Inform them about the loss of your card. You must also provide the operator with your registered mobile number, which must be the one linked to your Ayushman card.
The operator will identify you using this mobile number. Once identified, you can request free treatment without the card.
