Ayushman Card Drive Begins in Uttar Pradesh: Apply Online from Home, Know Benefits and Required Documents

The Uttar Pradesh government has launched a special month-long campaign to help eligible citizens generate their Ayushman Health Cards under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY). The drive began on 25 November 2025 and will continue until 25 December 2025, with the aim of ensuring that every eligible family member gets their own Ayushman card and can access free treatment whenever needed.

The campaign is being conducted simultaneously across all districts in Uttar Pradesh through both digital registration and offline camps. Special priority is being given to citizens above 70 years of age, who are considered the most vulnerable and often face challenges in accessing healthcare services.

Purpose of the Special Campaign

The state government has instructed district authorities to organize camps in areas where families have been left out so far. The goal is to expand the coverage of the scheme and ensure that no eligible person misses out on cashless treatment facilities.

Citizens can also contact their ASHA worker to check whether their names are included in the beneficiary list.

How to Apply for Ayushman Card from Home

The government has enabled a fully digital application process, making it easy to apply from home without visiting a camp.

Step-by-step process:

  1. Download and install the official Ayushman App on your mobile phone.

  2. Select preferred language and log in using your mobile number and CAPTCHA.

  3. Choose the PM-JAY Scheme under the Beneficiary section.

  4. Select your State and District, then enter your Aadhaar number.

  5. Check the list of family members; those without a card will display “Authenticate”.

  6. Tap on the member’s name and complete Aadhaar authentication, OTP verification, and e-KYC by uploading a photo and relationship details.

  7. After verification (done within about one week), download the Ayushman Card from the same app.
